WEST CARROLLTON, Ohio (WDTN) – The Occupational Safety & Health Administration (OSHA) is investigating what appears to be a deadly industrial accident in West Carrollton.

Medics and officers were called to Certified Heat Treating on Infirmary Road at 12:33 Monday morning after a man was found dead inside, the coroner told 2 NEWS.

The victim was identified as 48-year-old James Mitchell of Vandalia.

An OSHA spokesperson told 2 NEWS they have no inspection records on file for the facility. OSHA says the company may have been exempt from inspections or even too small for inspections.
