LANCASTER, Pa. (WGAL) A paperwork mistake is being blamed for a Pennsylvania inmate being kept in prison for 18 months past his release date.

A statement from the Lancaster Office of County Commissioners says the inmate was scheduled to be released on Nov. 25, 2014, subject to a parole plan, which was put in place on Dec. 1, 2014.

However, in the interim, new charges filed against another inmate who had the same last name were mistakenly placed in the wrong file, delaying the other inmate’s release until late in April 2016.

According to the statement, the other inmate was also held because the files had been duplicated and placed in both inmates’ folders.
